Fees:
_______ $ Security Deposit (1 week worth of Care) To be refunded in 21 business days from the end of the last billing cycle if a credit exist after all invoices have been paid.
_______$60.00 (maximum 1 ½ hours) Sunrise/Fluff & Tuck. This is a service to start (before noon) or end the day (after 6pm).
_______$40.00/minimum charge for care or services rendered from two to four hours
_______$35.00 per hour for care or services rendered for five-ten hours per day paid weekly after care is
delivered.
_______$33.00 per hour when you receive 70 hours or more a week, paid weekly after care is provided.
_______$32.00 per hour when you receive 70 hours or more a week paid 7 days in advance.

_______$95.00/visit/hour for RN
_______$____ per hour for the care of two people– This rate is based on the level of care of the care recipients.
LIVE-IN RATES
_______$15.95/hr. Level 1: The care recipient is ambulatory, does not require hands-on assistance for personal care.
_______$16.95/hr. Level II: The care recipient is able to transfer with the assistance of one person, may use a DME and may need hands-on care assistance for personal care and restorative care.
_______$19.00/hr. Care provided for two people in the home
